Dear FOIA Officer:
Pursuant to the Illinois Freedom of Information Act, 5 ILCS 140, I request copies of the following public records:
- All 10-year health/life safety surveys submitted under 105 ILCS 5/2-3.12 from January 1, 2000 to present — district name, RCDTS, submission date, approval/denial date, decision, reason if denied, total estimated deficiency cost, number of deficiency items, Regional Superintendent who reviewed.
- All amendments to approved surveys from 2000 to present — same fields plus amendment number and description of work added.
- Every application to issue bonds under 105 ILCS 5/17-2.11 from 2000 to present — district name, RCDTS, application date, bond amount requested, purpose, survey number, approval/denial date, decision, reason if denied.
- A list of all districts currently overdue for their 10-year health/life safety survey — district name, RCDTS, date last survey approved, date next survey due.
- Dollar value of health/life safety work approved through surveys or amendments but not yet completed, by district.
I request this data in electronic format (Excel or CSV preferred). If any records are withheld, please specify the statutory exemption for each record withheld.
